With APCM, you’ll have a dedicated nurse care coordinator who helps you manage your health between visits, stay on track with your care plan, and connect with your provider when you need it. It’s proactive, personalized care designed to keep you feeling your best and help prevent serious health issues before they start.

How Advanced Primary Care Management Works
Once enrolled, your provider will recommend the best type of monitoring based on your health needs and create a customized care plan just for you. You’ll stay connected in one of two simple ways:
Online: The free and secure Baptist OneCare© MyChart app allows you to easily share health updates at home. You’ll receive regular prompts to answer simple questions about how you’re feeling. Your responses go directly to your APCM nurse coordinator, who reviews them and communicates if anything needs attention. If necessary, we’ll help connect you to your provider for timely care.
Direct communication through scheduled phone calls: An APCM nurse coordinator will contact you regularly by phone to monitor your progress and support your ongoing care.

How do I enroll in Advanced Primary Care Management?
You will be contacted by phone to enroll. Simply give consent at the time of the call.

Will these services be covered as wellness or preventive care services through Medicare?
No. Medicare does not include APCM as a preventive care service, so deductible, co-pay and coinsurance standards may apply.

Will I have to pay anything?
Depending on your Medicare plan, there may be a minimal monthly cost. Once your deductible has been met, typical monthly out-of-pocket expenses are less than $10. In some cases, if you have a supplemental insurance policy, this cost may be covered at 100%.

How often will I be charged for the APCM services?
You can only be charged once per calendar month.

How do I see my individualized health care plan?
A copy will be provided to you via mail or your electronic health record via Baptist OneCare© MyChart, depending on your preference.

How do I contact the APCM care coordinator nurse or 24/7 emergency line?
During normal business hours, Monday – Friday, 8 a.m. – 5 p.m., call and ask to speak with your care coordinator.

If you are calling outside of normal business hours, please call your provider’s clinic directly and the call will be directed to the on-call provider.

Can I change my mind about receiving these services?
Yes, you can opt out of the program at any time by calling and communicating this request to your APCM care coordinator. The cancellation will be effective at the end of that month.
